This is the survey instrument accompanying the paper: Two Sides of the Shield: Understanding Protective DNS adoption factors. We used Foggs' model with the constructs: Motivation, Ability, and Trigger to inform the survey. We also present a description of Protective DNS based on literature review to determine likelihood of adoption of such a service.
